The aim of this project is to improve the recording and capture of patient co-morbidities, diagnosis and the care delivered during a patients episode of care.
This will contribute to improving the Trust’s Hospital Standardised Mortality Ratio by accurately reflecting what our expected number of deaths should be given our patient mix, basket of diagnosis and treatments.
