I am currently employed as a Discharge Case Manager for North Bristol NHS Trust. Within my role I am tasked with supporting clinical areas with the management of complex discharges. I also provide signposting for clinical areas and teaching around discharge procedures.
We are also responsible for managing delayed transfers of care, reduction in length of stay and patient flow.
I am a Registered Nurse by background and previously worked as a Staff Nurse in older person complex care. I have interests in gerontology, older people nursing and legal aspects within care of the elderly i.e. deprivation of liberty, power of attorney etc.
