90 Day enhanced care programme

Our trust is currently involved with a 90 day enhanced care programme run by the NHS Trust Development Authority http://www.ntda.nhs.uk/

This programme looks to improve care for our most vulnerable patients through carefully identifying and providing one-to-one care to those who need it most.

We are one of only a handful of trusts across England participating in the programme.

It involves us collecting data about the level of care our patients require and taking relevant steps to provide that level of care. The NHS Trust Development Authority is hoping to roll out this scheme to all NHS Trusts in England if successful during its pilot period.

The programme has so far enabled us to track patients at risk of falls and ensure relevant safeguards are put in place. This has seen a decrease in the amount of falls.
