At Home Prototype findings
61.3% of referrals assisted and directed away from council services by locality team working in the community before support planning
28.6% of clients who received support planning chose direct payments (an increase of 20% compared to baseline)
90.5% clients / carers were satisfied 100% of staff involved in the prototype believe in the principles

Case Study 1 X a 46yr old Adult with learning disabilities homeless living in Council’s temporary accommodation moved from his flat for his own safety by the police earlier this year - things stolen from his flat by drug users from the local area and he was taken to the cash point to take out money for his ‘friends’ on many occasions and physically assaulted.
Actions Emptied his old flat, he had not had access to belongings for over a month
Found X a new flat with the Housing Trust, helped set up tenancy, payment of bills and moving in
Put in 3 hours per week with Community Support from My Options to help with shopping, bill paying and tenancy support
Set X up with 1 day a week at the Skills and Enterprise Hub for 20 weeks to learn new work based skills.
X has recently moved to extra care X will have support with shopping, reading letters and cooking food . More importantly he will have a safe clean home with staff present at all times to help him when needed. They can also monitor who visits Xs flat and offer support as appropriate
X has been offered a voluntary job working at the restaurant at Extra care, he will get a free meal for helping out. X has been supported to sign up for a reading and writing class, and will now do this every week
