Partnership working between the NHS and local goverment

Have you seen this new video that NHS England has made with support from the District Councils Network, about partnership working and integrating care?

The video tells the story of a chance meeting between a District Council chief executive and a GP resulted in a health coach pilot going District wide with the potential of benefiting thousands of patients.

In the video we see how Angela, who eighteen months ago lost her husband Allan after 53 years of marriage, was supported through the health coach referral scheme, and an example of successful integrated care.

The video aims to help patients and the public understand better how the NHS and local councils can work together better to improve services.

The work described in this video has been led by Wealden District Council, Herstmonceux GP John Simmons and Freedom Leisure.
