It is centred around Peggy, aged 89 years, who has just returned home from hospital.
Her GP Ray has visited Peggy at home and recommends that she should be supported by our Proactive Care team.
The film includes:
• An introduction to Peggy.
• A multi-agency multidisciplinary meeting with Peggy’s GP to review her whole health and care needs.
• The appointment of Peggy’s key/case worker to ensure all services available to Proactive Care are involved in Peggy’s care.
• How Proactive Care supports the frail elderly and people with long-term conditions to retain independence in their own homes for longer and avoid admissions to hospital.
Proactive Care includes a number of agencies including Sussex Community NHS Trust (led provider in Coastal West Sussex), West Sussex County Council adult social care, GPs, Sussex Partnership NHS Foundation Trust (mental health), South East Coast Ambulance NHS Foundation Trust, local commissioners and hospital providers.
