My name is Graham Wilson. I am an occupational therapist by background. Since September I have been part of the Lincoln Home care service.This involves working across 22 care homes across Lincoln City.
The aim, through the implementation of the comprehensive geriatric assessment, is to take a proactive response in meeting current and likely future needs of the most frail and dependent members of community.
To date we have seen almost 70 residents and their families.
Our work ranges from cognition assessments ,mobility reviews, anticipatory care plans and working to achieve a good death for residents within their home setting.
Work is challenging but rewarding daily.
