A quality improvement project was run in Kent to improve communication between a community health trust and an acute hospital. The QI project was run by Kent Community Health NHS Foundation Trust (KCHFT).
At the beginning, colleagues at KCHFT said they felt communications between themselves and Maidstone and Tunbridge Wells NHS Trust were good, rating this at 53%, but they wanted to raise this to extremely good and 75%.
