End of life education

Out project aim was to improve and better ensure timely identification of patients approaching end of life and to ensure patients die in the appropriate place compassionately and without unnecessary delays.

Improving this aspect of care will have an indirect benefit of improving our mortality rates through the accurate recording of expected deaths.

Key tests of change (action) taken:

- Individual care plan monthly audits

- Staff Survey – to understand their learning needs

- Train the trainer approach to Sage and Thyme

- Updated training on wards and departments
