End of Life Care

End of Life Care Volunteers at SaTH

After a wonderful visit to Aintree Hospital hearing all about their EOLC Volunteer Service & how they set it up, showing us what good looks like, we returned wanting to do it even better. After lots of planning the 1st & 2nd cohorts were trained with a 3rd planned for Sep 2018.The End of Life Care Volunteers go through our main volunteer office & once they have received their volunteer training programme & relevant checks they join a cohort and receive their EOLC training with our fabulous Jules Lock Lead EOLC Volunteer & Jules Lewis Lead Nurse for EOLC.We cover the Swan Scheme, Communication, Mouth care, Safeguarding, Support and so much more. They meet key people in the Organisation & other volunteers. They receive a volunteer handbook with job description, useful contacts, maps etc. EOLC Volunteers work closely with the EOLC Service to support patients and their families at the hardest of times by offering companionship to patients who have few or no family or to allow family to have a well deserved break & look after themselves. End of Life Care Marriages in a Hospital setting

Shrewsbury and Telford Hospital NHS Trust Staff have been working hard to have a clear process for when an End of Life Care Patient wishes to get married in Hospital.Flowcharts have been designed for both Ward Staff and the Chaplaincy service to follow to ensure the process is done without confusion and nothing is forgotten at this special and emotional time, this includes required documentation for special licence & contact details of people who will be able to assist in and out of hours. Both religious and civil services can be offered depending on the couples preference. The end of life care team have created a wedding bag to assist the ward staff in making this day as special as possible, this includes bunting, a ring box, a wedding card, champagne glasses and fizz, confetti, pen, paper, a keepsake gift and the flow charts on the process.Together we made it happen because it's the right thing to do as we have one chance to get it right. You can read how they did it here

The film tells the story of Molly, her daughter and the care package around Molly that allowed her to remain at home and die peacefully. It’s a great story of what happens when the system works well and families talk in advance about a loved one's wishes
