The NHS North West Commissioning Support Unit (NWCSU) is currently running a pilot with South Cheshire GP’s utilising ‘Personalised Health Communications’ (PHC)
In brief, PHC uses technology to deliver highly personalised VIDEO communication via either a text message or email directly to patients providing them with: timely, relevant and actionable information and advice.
The PHC videos contain data relating to the patient themselves, enabling us to communicate about THEIR condition and not generically about A condition.
The degree of personalisation also extends to patient’s language preference and the ability to signpost people to services based upon their post code and cultural preferences.
Applications for PHC include:
· Chronic disease self-management
· Weight loss / obesity management
· Smoking cessation
· Low level mental health support
· Carer support
· Maternity support
· Medicines management
· Care pathway signposting
