We often talk about clinical commissioning evolving into a system that is focused on patient wellbeing as well as illness, and how commissioning for patient outcomes is a key part of how CCGs work on a daily basis, and this report shows just how CCGs are putting those words in to practice.
We all know that there are very real immediate pressures that need to be dealt with, but we also know that to ensure a sustainable NHS we have to plan for the future.
What we see in 'Delivering a healthier future' is CCGs looking forward and rising to the challenge of making long-term transformative changes for the benefit of patients, local populations and the NHS itself.
Among the case studies demonstrating work on prevention and early diagnosis, there are examples of CCGs:
• Proactively working with older people living with frailty to prevent A&E attendance and hospital admissions in South Worcestershire
• Supporting people to prevent and manage Type 2 diabetes in Blackpool
• Using peer educators to improve cancer awareness throughout the borough in Camden, London
• Working with the voluntary sector to ensure that people who wouldn’t normally access health services are getting care such as flu vaccines in South Kent
• Using social prescribing to improve health and wellbeing in Gloucestershire
• Focusing on atrial fibrillation as part of comprehensive efforts to prevent strokes in Barnsley and West Hampshire
