Rated ‘Outstanding’ by the Care Quality Commission (CQC) in February 2018 the service, based at The Gardens on the Diana Princess of Wales Hospital site, supports older adults in North East Lincolnshire who are elderly and experiencing mental health illness or memory problems such as Dementia. They offer specialist support to service users and carers within their own homes or ‘Konar Suite’ inpatient care facility.
The awards come from the Care Coordination Association (CCA) which has a long history of recognizing best practice in health & social care across both teams and individuals.
The service was nominated in both ‘Innovation to Support Service Development’ and ‘Embracing Service User/Carer Involvement’ categories.
The NAViGO projects that have scooped the honours for the service involve:
• Increasing service user participation in care planning – demonstrating real service user/carer involvement in an aspect of the care process.
• Introducing the new role of Discharge Liaison Social Worker, to ensure those who commission aftercare are integrated into the Inpatient team.
This improves engagement and effective working with service users, carers and clinicians. Konar Suite Clinical Team Leader Freedom Nwokedi, said the new challenge was all about embedding the importance of service user and carer involvement into patient care and care plans. “The team have worked hard to embrace this project and have all completed the bronze award in service improvement during this time which they should be tremendously proud of. We feel strongly that our service users are more empowered to take part in the care plan process than previously and we will continue to champion service user involvement irrespective of Diagnosis.”
