Announcing new NHS Elect Improvement Networks programme

We’re excited to announce a number of new programmes from Improvement Networks @NHSElect, providers of the award winning Acute Frailty Network (AFN), Ambulatory Emergency Care (AEC) Network and Specialised Clinical Frailty Network.

The team at Improvement Networks @ NHS Elect have worked with hundreds of clinical teams across the UK and internationally, to support colleagues with co-production of improvements to improve quality of care for patients. Our clinically led networks, with experienced teams of QI practitioners and analysts, have a wealth of experience. Our programmes are designed in line with the tried and tested ‘collaborative improvement’ model with enhanced support to local teams to improve services and models of care. Collaboration of teams aiming towards a shared vision is facilitated through sharing their experiences at networking events.

Details of our programmes are given below, including new additions for this year; Peri-operative Care for Older People undergoing Surgery (POPS) Network and the System Wide Frailty Network (SWFN) . Surgical Ambulatory Emergency Care (SAEC) Network

This improvement network follows the tried and tested AEC Network model to maximise same day emergency care. Each cohort runs for a period of 12 months. The SAEC Network is designed as a collaborative and teams from health communities sign up to work through improvement plans together. The programme is clinically led by Mr. Arin Saha and Mr. Simon Weaver and supported by senior NHS leaders who work with sites through their improvement journeys. As well as improving surgical pathways and systems, the programme includes training and support to undertake measurement for improvement and co-design with patients using experience based design.

A webinar was delivered by Arin and Simon in May 2021 to share the surgical SDEC model and the recording can be accessed here.

AEC Accelerator programme

The AEC Accelerator is a six-month programme of tailored support to help operational teams understand the effectiveness of their AEC model and realise opportunities to further maximise same day emergency care, shifting more overnight in-patient episodes to same day emergency care.

Virtual Wards programme

Our new Virtual Wards programme offer, is focused on working with teams to design and set up effective and efficient virtual wards outside of the acute hospital setting. Improvement Networks @NHSElect has worked with over 200 clinical teams to support the adoption and spread of same day emergency care. This has included working with colleagues to review systems, identify areas for improvement and create pathways to support patients outside of hospital, meaning we are well-placed to support trusts and clinical teams to successfully establish, implement and maintain virtual wards.

We recently recorded a podcast on virtual wards “Creating Virtual Wards on the Shoulders of Giants”, delivered by Professor Simon Conroy, Dr Vincent Connolly and Professor Daniel Lasserson. The podcast can be accessed here. POPS Network

Increasing numbers of older people are undergoing surgery and adverse clinician-reported, patient-reported and process-related outcomes are common within this patient group. A new approach to the Peri-operative care of Older People undergoing Surgery (POPS) has been developed by leading clinicians in the field over the last 17 years, with extremely good results. The POPS service has improved outcomes by integrating geriatricians into the care pathway of older surgical patients and demonstrates that preoperative CGA and optimisation reduces postoperative complications and length of stay in older people undergoing elective orthopaedic surgery.

There is now a bold ambition to share this work and drive the adoption and spread of this service model across the NHS. The POPS Network will work with NHS organisations to help them understand and adopt the POPS model locally as part of their work on redesigning and restoring elective care, and ensuring services are designed to meet the needs of older people. We will support teams during a six-month collaborative programme of learning and development events. We launched POPS Cohort One Launch Event on 8 June 2021 and a second cohort will launch in Autumn 2021.

System Wide Frailty Network (SWFN)

There is a pressing need to develop systems to provide care closer to home for older people living with frailty, both to minimise unnecessary urgent care episodes and improve outcomes. Planning services across local systems to meet the needs of this group of vulnerable patients is key to providing better and more resilient NHS care.

The SWF Network supports you to develop a whole system approach to the care of older people. This programme will help with the design and delivery of care pathways in response to the Ageing Well policy and aims to support teams to improve care and outcomes for older people. A twelve-month programme, the SWFN Launch Event will be in early September 2021, but we’re keen to start working with sites as soon as possible. AFN Optimiser Programme

The AFN Optimiser builds on our popular Acute Frailty Network programme and offers tailored support to help operational teams to understand the effectiveness of their frailty model to realise opportunity for improvement. Site support from the Acute Frailty Network (AFN) team is provided to help teams gain a deeper understanding of local challenges, including use of data and a casefile review to determine areas of focus for further improvements in the Acute Frailty service.

Two workshops are provided during the six months and are based on themes that emerge during the review, with content tailored to your organisation and locally delivered. This helps to maximise attendance whilst minimising cost and disruption to you. You will be allocated a QI Associate from the national team who will work with you in implementing and measuring your improvements. This is a rolling six-month programme so organisations can start the programme at any point during the year.
