Introducing an Integrated Provider Hub enables us to have an outcomes based contract across care pathways (regardless of who is providing the service) rather than commissioning episodes of care in a potentially fragmented fashion.
This approach allows commissioners to clearly articulate the desired outcomes for the population and enables all providers to work collaboratively to transform services to deliver these outcomes. Ultimately, this ensures that care is provided to the patient at the right time in the right way by the right people.
The Integrated Provider Hub is a partnership between Cheshire and Partnership Trust, NHS Western Cheshire CCG and Cheshire West and Chester. The Integrated Provider Hub works with other third sector, voluntary and independent providers within the mental health pathway.
Everything the Integrated Provider Hub does has an impact on service users; its work is about ensuring our resources are used in the most effective way to deliver care in the right place at the right time. We recognise that it is vital to involve service users as partners in the development of this model and so we do have service user representation on the programme board which oversees the Integrated Provider Hub and in the workgroups developing care pathways.
