How do we tell the NHS story of integrated care and services?

Over the past few months, NHSE have produced a set of resources to help us all to tell our story about why the NHS and councils are joining up health and care through STPs and integrated care systems.

We are promoting these to important national groups including politicians, professional bodies and charities, and through our national staff and patient networks.

 

These resources include:

1. a new briefing Breaking down barriers to better health and care;

• an infographic setting out why services need to change (attached);
• web pages exploring in detail the firstintegrated care systems and what they have done for local areas;
• we will also remind people of the range of videos showing how partnership and integration are making a difference, which include:

• • joining up care and support for older people in Dorset
  • preventing unnecessary hospital admissions by offering better support in Lancashire care homes
  • helping people in South London to live independently, in partnership with local charities
  • local councils and GPs working together in Sussex to combat isolation and reduce health risks
  • saving lives by transforming services for people in Greater Manchesterwho have had a stroke

We hope these will help people to understand our work better – please share them with local communities, staff networks and others who would find them useful. We also hope you will be able to adapt the content or produce similar material locally – let us know if you are considering this and want to discuss your ideas with someone from our team. The briefing builds on recent written evidencefrom NHS England and NHS Improvement to the House of Commons Health and Social Care Committee’s inquiry on integrated care (organisations, partnerships and systems), which will publishing its findings and recommendations yesterday.
