Supporting people to become more involved, connected and active in their communities

Integrated Plus is being delivered by Dudley Council for Voluntary Services (Dudley CVS) as part of the integrated care model, being implemented by Dudley Clinical Commissioning Group and Adult Social Care.

This new innovative and exciting approach will:

– Support implementation of integrated care model being implemented by Dudley CCG and DACHS

– Support frequent attenders of medical services to become more involved, connected and active in their community

– Enable the voluntary and community sector to play an active role in integrated care

– Raise the profile of the voluntary and community sector VCS to health clinicians
