So how did the Acute care collaboration Vanguards get selected?

There are 13 Acute care collaboration vanguards which link together local hospitals to improve their clinical and financial viability.

In September, the new care models' team hosted an event to select the successful acute care collaboration vanguards.

The  selection process was rigorous, involving workshops and the engagement of key partners and patient representative groups. Each applicant gave a presentation which was followed by a short question and answer session.

You can find all of the videos here but we thought we'd feature the presentation from Salford and Wigan Foundation Chain (Multispecialty chain) who are working together with Devo Manc partners to create a new healthcare group. This will help them to increase the pace at which they are able to make improvements which will result in better outcomes for their patients and increases in productivity. They are working in partnership with commissioners and provider organisations across the region to create an Integrated or Accountable Care type organisation and a single service model for elective and specialist services.

Have a look at the video - really redefining how organisations work together to provide seamless care
