Their pledge for FAB Change week was a simple one but one that informs our patients so much and ensures they are involved in their care decisions.
In Lincolnshire Community Health Services Trust we have a document called the 'Ticket Home'
This document gives the patients information about who will be involved in their care and what we will do to aim for discharge home and also gives them a predicted date of when this will be.
Janine and Emma pledged to ensure this document was given out to new patients.
They did this by giving it to them following every MDT on a Wednesday and inform them and involve them in the plan of care.
