A frailty clinic was piloted at Edenbridge Memorial Health Centre with the aim of helping older people to access help and support quicker.
During the trial, Comprehensive Geriatric Assessments were carried out at the clinic for 20 moderately to severely frail non-housebound people. This resulted in people being able to have their assessments sooner, resulting in care being proactive rather than reactive.
