My pledge is to support and work with teams throughout the Trust and local health economy to continuously improve End of Life Care for our patients and their loved ones.

Led on End of Life Improvement Plan using five key lines of enquiry. Facilitated the following improvements:

- End of Life Champions in all clinical areas

- Quality assurance templates for each of the KLOE

- Introduced role of Ceilings of Treatment Nurse Advisor

- Implemented Treatment Escalation Plans

- Increase of palliative care service to 6 days per week

- Implementation of end of life magnets to annotate patients on End of Life individualized care plan/anticipated last days of life

- End of Life Trust newsletter to share the learning and raise awareness of resources

- Collaboration with two local CCGs to ensure uniform assessments for fast track

- Raised profile of Specialist Palliative Care Team and links with chaplaincy services
