Poetry in Motion; bringing Local Care to life in Kent and Medway featured image

The local care model is being developed as a key element of the Kent and Medway Sustainable and Transformation Plan (STP).

Getting local care right for patients means health and social care professionals truly working as multi-disciplinary teams (MDTs) which support primary care working at scale (populations of 30-50,000).

What is an MDT, was developed from a poem by Cathy Bellman and is now a film where local staff and patients show off their acting skills to bring it to life.
