Manage your Health and Wellbeing goals with Health Fabric

Digital Health experts, Health Fabric, have released a new mobile application that enables anyone to achieve their Health and Wellbeing goals.

There is no quick fix to a healthier life. An understanding of your individual needs is required plus additional knowledge to manage any long term condition, or just remain healthy. Health Fabric can enable users to manage their own health/condition, whilst also giving family and friends, carers or clinicians (if invited) to work together to create better outcomes for them, improving communication and care co-ordination through its interactive features.

What makes the MyHealthFabric app unique is its simplicity for all to use, regardless of their personal needs. Health Fabric also have a library of Health and Wellbeing plans that have been created in collaboration with a number of accredited organisations, available to be downloaded FREE and personlised to your needs. Users are also able to set notifications, record actions and track their overall progress – this can then be shared with their family, friends, carers and healthcare professionals (if invited).

Other key features on the app include a mood diary, medication, healthcare measurements, incident plans, summary reports and more.
