Looking to improve how patients are cared for at the end of life?

Want to create an environment in which staff feel confident having end-of-life care conversations around patients’ and their families’ wishes?

Following the success of the 2016 ‘Living Well to the Very End’ cohort, The Point of Care Foundation is recruiting 20 multidisciplinary clinical teams from health and care provider organisations across the UK, and is inviting you to join.

Funded by The Health Foundation, ‘Living Well to the Very End’ is an ambitious programme focusing on the care of people at the end of life, in generalist (rather than palliative care) settings.

The programme uses the patient and family centred care methodology, which is truly unique in that it draws on patient shadowing to help teams understand the experience of patients and families, and to create the urgency for change.

Head of Improvement and member of the Q initiative, Bev Fitzsimons, will be leading the programme with support from a faculty of improvement coaches.

Applications close on Friday 31 March.
