Local Care is the key element of the Kent and Medway Sustainability and Transformation Partnership (STP) model. Getting local care right for patients means health and social care professionals truly working as multi-disciplinary teams (MDTs) which support primary care working at scale (populations of 30-50,000).
What is an MDT, was developed from a poem by Cathy Bellman and is now a film where local staff and patients show off their acting skills to bring it to life.
