Integrating psychological, social, and diabetes care for patients with poor glycaemic control

The Three Dimension for Diabetes (3DFD) service at King's College Hospital is a model of care that integrates psychological and social interventions into diabetes care for people with complex psychological needs.

The service supports people struggling to cope with their diabetes and social problems like homelessness, unemployment and debt.

Since 2010, the service has completed two successful pilot phases and made significant improvements in these patients' glycaemic control, psychological status, and health & social care use.
