Improving Handover from Emergency Admissions Unit (EAU) to Ward - Quality Improvement Project at Yeovil Hospital featured image
Patients would leave the Emergency Admissions Unit (EAU) for a medical ward and have a verbal nursing handover on transfer; however the handover to ward doctors is solely what they glean from the medical notes, which can vary in quality. Doctors reported difficulty in finding diagnoses and management plans.

We aim to improve communication between medical teams to enhance the safe transfer of patients from EAU to medical wards.

Our chosen action was to design and test a proforma that the EAU doctors would be able to fill in quickly and easily, and would clearly impart key information to the ward doctors.

We went through several modifications to the form design based on feedback from the ward doctors and are planning education sessions for the EAU team to help implement the proforma effectively across the unit.
