HARM FREE CARE PROGRAMME: FALLS WORKSTREAM: Using a Quality Management systems approach

Background: Reducing harms from falls is key priority of Trust Quality strategy.Nice Falls guidance highlighted gaps in our Falls proforma that needed urgent attention.

Aim for a Falls rate at 4.0 from baseline of 4.8 by December, 2024.

Key change drivers were enhancing falls profile through communication and engagement. Identify high risk falls patients using BAYWATCH

Change ideas tested on 4 pilot wards included falls digital proforma, MDT engagement sessions to understand what works and what were the barriers

Measures and Impact - we achieved our aims through an improvement in falls risk assessment compliance of 92% and falls rate improved from 4.8 to 4.1 lower than national average of 6.6!

Key learning:QI gave me the knowledge and skills to deliver changes . It has made the falls service more productive and empowers us to be our best.It has taught me how a system can be made effective and efficient at the same time.
