For many months now we have been working hard behind the scenes getting ready for a very special Fab Change 70 17th,
18th & 19th October it's time to celebrate and share our plans.
The End of Life Care Service have been working with ITU, Bereavement, Mortuary, Community teams & more to launch a new service to get ITU / HDU EOLC Patients home for EOLC when home is their Preferred Place of Care & when safe & kind to do so, this will officially start in Fab Change 70 & be developed in the coming months to cover all areas within Shropshire.
Two Fab Red to Green Trackers & EOLC Champions Louise & Chantelle have worked with us to implement a Swan Scheme out of hours cupboard to allow staff to obtain moon lights, CD player & cds, new improved swan memory boxes for expected deaths with helpful contents & now includes forget me not seeds, lip balm and 2 x relative vouchers for hot drink & cake, swan paper bags with contents for unexpected deaths, fleece blankets, information leaflets, message cards and much more, making sure staff have the supportive information and equipment when they need it at the hardest of times. All wards are stocked with many of these items. However, stocks can become low out of hours & at weekends. This will be shared with all staff before Fab Change 70 & launched on the 17th October.
We have almost got to our target of 20 swan rooms / pop up swan rooms and we will be officially opening many on Friday the 19th along with a celabrate and share Coffee & Cake morning at RSH & afternoon Tea & Cake pm at PRH.
We are also launching our new improved version of our End of Life Plan making sure all patients diagnosed as being in the dying phase are supported by an individualised EOLC plan following important conversations by the senior medical team, this will allow staff to make sure the right care & support is delivered at the right time, making sure nothing is forgotten & what's important to the dying person & their family is not only listened to, acted upon with support from the EOLC Service, including our award winning EOLC Volunteers of which we now have 15 they are all amazing & if that's not enough we will be making further improvements to care after death.
So on Friday the 12th I will attend a stand up / huddle at SaTH to share our plans & then let Fab Change 70 begin. Inviting all staff to come and share their fab stuff & of course I will be encouraging all staff to be brave & join me in no email Wednesday on the 17th October.
