My September share is the End of Life Care relative leaflet, this information has been in the back of the end of life plan used at SaTH for some years now.
However, Rachel Preece a sister at the hospital who worked within the EOLC Service for 12 months on secondment felt it also needed to be in a nice easy read leaflet too, so that's what we did with the help of our fab web development team.
Not only do all wards now have copies in their EOL & Bereavement Care box files, from next month they will also go in our new improved swan memory boxes along with other useful information, tissues, swan pen, swan paper, swan property bag, jewellery bag, forget me not seeds and 2 x EOLC relative vouchers for tea and cake on us.
The improved boxes will be packed with kindness by our EOLC Volunteers.

This will all happen in time for fab change 70.
The little things are the big things, because we have one chance to get it right.
