The Discharge Assessment team is a therapy lead service, based in the Emergency Department at the Royal United Hospitals, Bath.

The attached poster highlights the positive impact of therapists working at the Front Door, to support hospital flow, and timely discharge for their patients.

The data in the poster reflects the service year April 2016-March 2017.

The Team are members of the SAMs network, and are keen to link with other front door teams to share ideas and develop services.
