The event celebrates quality improvements in the NHS across the north of England, focussing on the amazing work happening in dementia. The Memory and Later Life team presented the changes in the pathway which they have developed to improve person centred approaches, efficiency and effective use of resource. They ensure that their focus is on the impact of dementia on the family; this follows the team’s successful #seethePERSON campaign.
David Storm, Senior Clinical Services Manager at CPFT said: “The service has piloted ‘virtual clinics’ and care pathways for early dementia, including specific carer and family interventions. This approach has been designed to improve patient/family experience, with diagnosis and post-diagnostic follow up. “It’s a fantastic achievement for the team to achieve such recognition, especially given the scale of other Trusts and services within the North of England. “It shows how passion and commitment to make a difference to people can transform services without increased resource, by focussing on quality as well as understanding what really matters to people who experience dementia and their families.”
The Memory and Later life service were runners up in the preventing/diagnosing well category.
