How a dementia pathway could be- Jack and Jenny's story

We’d like to bring to your attention a new animation that was commissioned by Doncaster CCG on behalf of Yorkshire and the Humber Clinical Network for dementia and developed by Inkwell Arts Media and Leeds Mind.

This short animated video highlights how a dementia pathway could be by telling Jack and Jenny’s story.

Dementia does not have a linear pathway but like many other life limiting illnesses and long term conditions, it does have a start and an end.

You can watch the video by following this link: http://www.youtube.com/watch?v=5h4IV1IVjM8

We hope you will support, promote and share this resource amongst your colleagues and networks.
