The partners include Derbyshire Community Health Services and Derbyshire Healthcare NHS Foundation.They are removing organisational boundaries and shaping new services that give local people the information and confidence they need to reduce their reliance on health and care services.
The vanguard’s community and personal resilience workstream is focused on working with communities to promote self-care through education and a greater availability of health information, and on training staff to talk to people in a different way and involve them in making decisions about their own health and care.
Its aim is to reduce people’s dependency on health and social care by motivating them to have greater ownership, control and understanding of their own health and wellbeing, and by creating strong support networks within local communities.
Workstream members include service leaders from community, voluntary and statutory services as well as local citizens.
This guide describes how the workstream was set up, its actions so far, the challenges faced and key learning points. It is aimed at other vanguards or organisations who want to put a similar workstream at the centre of their work with ‘place based’ communities.
This can be used to support the ‘place’ requirement in the sustainability and transformation plans (STPs).
