Clinical and Non-Clinical posts supporting patient flow in AMU

Our Acute Medical Unit (AMU) at Epsom and St Helier have ensured a coordinated approach to ensuring patients receive the right care, in the right place, at the right time.

By recruiting a band 5 non-clinical co-ordinator and a band 7 clinical nurse navigator, patient flow is maintained through the unit, supporting the rest of the hospital.

Some of the key areas which contributes to the success of the roles is:

* The roles work together to run the MDT board meetings to ensure operational elements of the patient journey are progressed in tandem with a patient's medical needs.

* Both roles liaise closely with the capacity management team, using operational expertise to ensure flow within rapid assessment area of the hub.

* Both roles liaise closely with external colleagues to ensure timely and appropriate discharge of patients back to their usual place of residence with appropriate support.

This model of clinical and non-clinical staff has supported the Trust to continue to maintain patient flow and deliver the right care, in the right place, at the right time.
