Enhanced health in care homes vanguards

In March 2015, six enhanced health care homes vanguards were launched as part of the NHS Five Year Forward View.

Since then, they  have been redesigning the way they deliver care for older people by joining up health, care and rehabilitation services.

The includes making better use of new technology, improving the support and training staff receive and working more closely with local voluntary and community groups.

They’ve also improved their connections with local NHS and care partners and increased the amount of time GPs and other primary care partners spend with residents.

Description

This film recognises the work and achievements of the six enhanced health in care home vanguards and tells their story so far.

Through a series of interviews with staff, residents and volunteers, the film showcases the impact new models of care are having on people’s health and wellbeing and how vanguards are achieving this.
